Grant Description
Description These funds will be used to provide necessary and critical case management to our homeless families in our programs. We provide professional case management to all of our families whether they reside in our transitional housing units or in our transitional housing units. Case management includes the following: <br/>- Family Stability Plan in collaboration with parents and their children<br/>- Goals for each family and tracking progress against those goals<br/>- Employment and Housing specialist to assist families in accumulating assets to be used for permanent housing (rental deposits, credit report repair, etc)<br/>- Referrals to community resources for employment, trauma counselling, nutrition, parenting skills<br/><br/>We use a Family Stability Plan to track the progress of our residents/guests. This plan includes specific goals and outcomes for each family. On a semi-monthly bases, families meet with their Case Manager to assess progress against their plan. For both programs, a measurement for success is the number of families moving into permanent and stable housing at the end of their stay. Historically, our success rate is 85%.. For the Family Shelter, metrics also include the Hope Scale Model which is used upon families entering the program and again when they exit the program. Additionally, both programs track household income and number of job searches for each family upon entering the program and exiting the program.
Used for These funds will be used to defray operational costs for our Transitional Housing program and our Emergency Family Shelter program. Specifically these funds will be used for providing case management services so families can become gainfully employed and move to permanent stable housing.
Benefits Our programs reduce homelessness in the area of Western Washington. We provide 20 transitional housing units and a capacity of 29 emergency shelter units (90 Beds) to the homeless thus reducing the homelessness in our area. Additionally, our families achieve gainful employment and are removed from welfare rolls in the community.
